Hospital Liaison (MSU)


Program Name: Medical Services Unit (MSU)

Works with the following population: Individuals with mental illness, developmental disabilities, or addictive diseases.

Days & Time: This position will work 40 hours a week. Monday through Friday. Most programs work from 8am to 5pm, with a one-hour lunchbreak.

Pay: $36,050 to $41,500 per year.

Job Responsibilities

The candidate will be responsible for:

  • Providing individual and/or group services as needed based on the ISP.
  • Functioning as the main point of contact and liaison between hospitals and CSUs. 
  • Following up with individuals who have been discharged from hospitals and/or CSUs to ensure a smooth transition and continuity of care. 
  • Utilizing case management strategies to educate and connect individuals to services. 
  • Utilizing a person-centered approach to meet the unique needs of each individual. 
  • All other duties as assigned.

Minimum Requirements:

Bachelor's degree in the field of social science or human services, or a high school diploma/GED and three (3) to five (5) years of experience providing behavioral health services.

Preferred Qualifications:

Three (3) to five (5) years of experience providing behavioral health services.
