Program Name: Medical Services Unit

Works with the following population: High Risk Individuals and frequent hospital discharges. 

Days & Time: This position will work 40 hours a week. Monday through Friday, from 8am to 5pm, with a 1-hour break.

Pay Range: $54,590 to $60,000 per year, depending on experience.

Job Responsibilities: 

Under general to limited supervision, the candidate will:

• Perform social services and clinical functions at an advanced level to individuals and their families. 
• Perform/Complete intakes, assessments/reassessments, and determining viability for other programs and services. 
• Receive training on the 1013 process and how to monitor moderate and high-risk individuals. 
• Other duties as assigned.
  

Minimum Requirements: 

You MUST have a master's degree in social work or counseling, and you MUST be fully licensed as an LPC or LCSW in the state of Georgia.
