Program Name: Medical Services Unit

Works with the following population: Male and female adults and children with a mental health diagnosis and/or addictive diseases.

Days & Time: This position will work a 40-hour week, Monday through Friday. Most programs work from 8am to 5pm, with a one-hour lunchbreak.

Pay: $38,325 to $40,427 per year

Job Responsibilities

Under general to limited supervision, the candidate will: 

• Prioritize continuity of care. 
• Identify individuals' healthcare, social needs, and support needs.
• Identify barriers to care and access to community support resources.
• Coordinate and collaborate services with internal and external professionals and community agencies.
• Schedule appointments and arrange referrals to appropriate healthcare providers.
• Educate patients and families about available resources.
• Advocate for individuals to ensure they receive appropriate services and support.

Minimum Requirements:

Bachelor's Degree in one of the following fields: Social Work, Counseling, Psychology, Sociology, Mental Health, or a related discipline -OR- one (1) year of full-time experience as a Social Services Technician. Must have one (1) year of experience working directly with the public. Must have knowledge of healthcare systems, community resources, and patient advocacy. 

Preferred Requirements:

Previous experience in healthcare, case management, or care coordination.
