Program Name: Intensive Case Management

Works with the following population: Individuals with a mental health diagnosis, developmental disabilities, and/or addictive diseases

Days & Time: This position will work 40 hours a week. Most programs work Monday through Friday, from 8am to 5pm, with a 1-hour break.

Pay: $65,000 per year

Job Responsibilities: 

Under general to limited supervision, the candidate will:

• Manage the daily operations of the Intensive Case Management department. 
• Conduct weekly team meetings and individual supervision. 
• Complete monthly reports at the agency and state levels. 
• Attend internal and external meetings. 
• Complete Biopsychosocial assessments, treatment planning/monitoring, transition and discharge. 
• Provide counseling therapy to clients requiring complex intervention.  
• Perform commitment determination and/or execute orders of involuntary commitments. 

Minimum Requirements: 

The candidate MUST have a master's degree in social work or counseling, and MUST be fully licensed (LPC, LCSW, LMFT) in the state of Georgia. The candidate MUST have experience with case management services, experience managing programs and supervising staff, and experience in behavioral health. The candidate MUST have a valid driver's license, as this position may require transporting of individuals.
