Program: DD Residential Services

Job Type: Full-Time (40 hours per week)

Location(s) served: New Horizons' coverage area.

Population served: I/DD and Mental Health.

Days & Hours: Monday - Friday, 8am to 5pm, but will be on call after hours and on weekends.

Salary: $58,000 per year

Job Responsibilities

Under general supervision, the candidate will be expected to:

• Perform nursing assessments.
• Document follow-up care and primary care physician visits.
• Conduct site medical room checks.
• Assist RN on site as needed.

Minimum Qualifications: Must have a valid LPN certification. Must have two (2) years of work experience in a hospital, medical office, long-term care, etc.

Preferred Qualifications: Mental health experience.
